*TUBERCULOSIS TESTING: You are required to have a TB skin test (PPD) or undergo a Quantiferon TB Gold blood test. These tests must be done WITHIN THE USA and within 6 months prior to registration. Please have the administering healthcare provider complete our PPD Recording Form (if performing a PPD). You may print this form from the Patient Self-Service section on our website at www.nuhs.northwestern.edu. Submit a lab report if Quantiferon done.
All deficiencies must be rectified by the start of classes to avoid a non-refundable $100 late fee and registration hold. Records may be submitted by fax to 847-491-8699. Upon receipt of required documentation, your status will be updated.
